  Student enrollment is conducted in the District Office Monday - Friday from 7:00 a.m. to 3:00 p.m.

To register, please bring the student's official birth certificate, immunization records, custody papers (if applicable) and 3 forms of proof of residency.

  Acceptable forms of Proof of Residency are:

(all must show parent's name & current street address in our district)

Copy of voter registration

Copy of motor vehicle registration

Copy of change-of-address form request submitted to the Post Office

Copy of valid Ohio driver's license

Copy of federal, Ohio or local income tax return

Copy of invoice for moving expenses

Copy of utility bill (current month)

(electric, gas, phone, cell phone, cable, sewer, water & trash, etc.)

Closing statement on house

Copy of rent receipt (current month) 

(receipt must show tenant's name, current address, landlord's name & landlord's phone number)

Paycheck stub (current month)

Insurance forms (health or auto)

Bank statement (checking or savings) - current month

Real estate tax statement

Enrollment Packets

 

ALL completed enrollment packets should be faxed to Noelle Murray at 937-962-4731 or mailed to:  436 N. Commerce Street, Lewisburg, OH 45338.  Noelle is the initial point of contact for all enrollments.  Enrollment packets will then be filtered to the correct building.  Should you have any questions, please contact Noelle at  937-962-2671.  Thank you!
